Princípios universais
Testa as substâncias (drug checking)
- Análise laboratorial (Kosmicare em Portugal): HPLC/espetrometria, identifica e quantifica. Sempre superior aos reagentes caseiros.
Set & setting
- Espera.
A balança é sempre a melhor opção.
O kit mínimo (barato, cobre a maioria)
How to read this guide
Dose scale
Each substance uses five levels. More is not better: more is more risk. Always start low and wait out the full come-up before even thinking about more.
| Threshold | First noticeable effects. |
| Light | Mild effects, a good starting point. |
| Common | The typical dose for most people. |
| Strong | Intense: risk and side effects go up. |
| Heavy | High risk. Only for experienced users, and even then not advised. |
Combination risk code
Universal principles
Test your substances (drug checking)
The most important harm-reduction step. In Portugal you can do it for free and anonymously (see resources).
- Reagent test kits: they change color depending on the substance. Marquis, Mecke, Mandelin, Simon's, Froehde, Ehrlich (for LSD/psilocybin/DMT). For MDMA use Marquis + Simon's + Froehde together. Limitation: they indicate presence, not purity or the absence of adulterants.
- Fentanyl test strips: dissolve a sample in water and dip the strip. One line = positive, two = negative. Essential for any street powder or pill. A negative is no 100% guarantee (they don't detect every analog).
- Benzo test strips: for pressed pills and stimulants, where new benzos show up as adulterants.
- Lab analysis (Kosmicare in Portugal): HPLC/spectrometry, identifies and quantifies. Always superior to home reagent kits.
Set & setting
Set (mental state: mood, mental health, intentions) and setting (physical and social environment). Anxiety, grief or trauma get amplified by psychedelics and dissociatives. Choose company you trust, avoid unfamiliar or very crowded spaces for your first times, and have an exit plan.
Dosing rules
- Start low, go slow. Take the bottom of the range, especially with a new source or batch.
- Wait. Most oral routes take 30 to 90 min to peak. Redosing before the peak (because "you don't feel anything") is one of the main causes of accidental overdose: the second dose hits on top of the first.
- Weigh it, don't eyeball it. The best money you'll spend on harm reduction: a milligram scale (0.001 g) costs ~€15 to 25 and solves this.
A scale is always the best option. Without one, you can get a rough idea with the capsule technique: open a capsule from a supplement with known mg (e.g. 200 mg of L-theanine), look at the volume of powder, and compare it with your substance in the same capsule.
The catch is density: different powders weigh differently for the same volume. Crystals and fine, dense powders weigh more; fluffy powders weigh less. Since L-theanine is light and fluffy, almost any drug will weigh more for the same volume. In other words, you're probably taking more than you think: assume it's more and start low. If you can, use a reference with a similar texture.
Never use this for narrow-margin substances, where a few mg can kill: benzos, GHB/GBL, 2C-B and the 2C family, and any powder that could contain fentanyl. With those, either you have a scale or you don't dose.
Insufflation (snorting): common rules
Applies to any snorted substance (cocaine, ketamine, speed, 2C-B). The nasal lining is fragile, and the damage adds up with repetition.
- Grind it to a fine powder. Large crystals cut the lining; crush well before dividing it up.
- Split the dose between both nostrils and alternate nostrils between sessions, to give each side time to recover.
- Your own gear, never shared. Invisible micro-cuts in the lining transmit hepatitis C (and potentially HIV). A clean straw costs cents.
- Saline nasal rinse only AFTER the burn has passed, never before or during: while the substance is absorbing, rinsing just drags it down your throat and irritates it more. Afterwards, the saline clears out the residue and helps the lining recover.
Hydration: the Goldilocks problem
Not too much, not too little. MDMA and other stimulants make you retain water (they release ADH): drinking too much dilutes your blood sodium and causes hyponatremia (nausea, headache, confusion, seizures, can be fatal and gets mistaken for an overdose). Drinking too little in the heat while exerting yourself causes dehydration.
Tolerance and cross-tolerance
Repeated use reduces the effect (the receptors adapt), which leads to higher doses and higher risk. Cross-tolerance applies within the same class: classic psychedelics (LSD, psilocybin, mescaline) share near-total tolerance. If you used 2 days ago, the same dose may deliver less.
Losing the magic: the danger of upping the dose
With MDMA (and also with stimulants and ketamine), heavy or frequent use can make the drug "lose the magic": it stops giving the euphoria and starts giving only the stimulant part (sleep loss, feeling wired, anxiety). It happens because the serotonin system gets depleted and downregulated, and with heavy use there may be some neurotoxicity. Classic psychedelics don't have this problem (their tolerance resets in days).
Supplements: the essential minimum
The minimum kit (cheap, covers most cases)
For stimulants and MDMA, if you only buy three things:
- Magnesium essential curbs jaw clenching and helps with protection. Cheap.
- Vitamin C and/or ALA essential a simple, affordable antioxidant.
- Electrolytes (or a sports drink) essential hydration, almost free.
Non-negotiables by substance (cheap and critical)
- Nitrous (N2O) → B12 is the most important thing and costs cents. Without B12, repeated use damages the nerves.
- Alcohol → B1 (thiamine) for heavy sessions.
- Stimulants and MDMA → magnesium + hydration.
The rest is a bonus
ALCAR, CoQ10, EGCG, vitamin E and company are for those who want to maximize and have the money. They are not mandatory. On each substance card, the labels show where to focus: essential recommended optional.

Classic psychedelics
LSD
PsychedelicOral, sublingual
Avoid with a history of psychosis or bipolar disorder
LSD
Avoid with a history of psychosis or bipolar disorder
- Test it with an Ehrlich reagent (purple/blue = indole present) + a fentanyl test strip.
- Set & setting and a sober sitter who knows what you took.
- Start low (50 to 75 µg your first time) and don't redose (the come-up is slow and deceiving).
- Bail-out plan: a benzodiazepine on hand (reduces the trip, doesn't fully block it).
- Don't drive for 12h+. Avoid it if there's a personal/family history of psychosis or bipolar disorder.
Doses (oral, in micrograms µg)
| Level | Dose |
|---|---|
| Threshold | 15 µg |
| Light | 15 - 75 µg |
| Common | 75 - 150 µg |
| Strong | 150 - 300 µg |
| Heavy | 300+ µg |
Duration
| Onset | 15 - 30 min | Peak | 3 - 5 h |
| Total | 8 - 12 h | After-effects | 12 - 48 h |
Long-term effects of abuse
- HPPD (persistent visual disturbances): rare, risk rises with frequent high-dose use.
- Can trigger latent psychosis in predisposed people.

Weigh it with a milligram scale
2C-B
Weigh it with a milligram scale
- WEIGH IT with a milligram scale. It is extremely dose-sensitive: a few mg make all the difference.
- Test it (Marquis turns blue/black) + a fentanyl test strip.
- Oral is safer and more predictable; snorted it hurts a lot and hits harder.
- Watch your temperature and heart rate in hot environments.
- Don't combine it with MDMA without cutting both doses way down. Sitter above 20 mg.
Doses (oral)
| Level | Dose |
|---|---|
| Threshold | 5 mg |
| Light | 10 - 15 mg |
| Common | 15 - 25 mg |
| Strong | 25 - 45 mg |
| Heavy | 45+ mg |
Duration (oral)

DMT
Doses (fumado/vaporizado)
| Nível | Dose |
|---|---|
| Limiar | ~2 mg |
| Leve | 10 - 20 mg |
| Comum | 20 - 40 mg |
| Forte | 40 - 60 mg |
| Pesado | 60+ mg |
Duração (fumado)
| Início | 20 - 40 seg | Pico | 2 - 8 min |
| Total | 5 - 20 min | Efeitos residuais | 10 - 60 min |
Efeitos a longo prazo / riscos
Set & setting primeiro.
- Integração (descanso, escrever, terapia) nos dias seguintes a sessões intensas.
Ayahuasca
DMT oral + IMAOOral
Washout de antidepressivos não é negociável
Ayahuasca
| Total | 5 - 10 h | Efeitos residuais | 1 - 8 h |
Efeitos a longo prazo / riscos
Exigem washout.
- MDMA e estimulantes (cocaína, anfetaminas) - síndrome serotoninérgica e crise hipertensiva.
- Tramadol, DXM, 5-HTP, triptofano - carga serotoninérgica perigosa.
- Outros IMAO e descongestionantes (pseudoefedrina) - crise hipertensiva.
- Vaporiza, evita chama direta.

Sit or lie down before the first hit
DMT
Sit or lie down before the first hit
- Sit or lie down before the first hit - the effect kicks in within seconds.
- Sober sitter for physical safety only. The person is gone for 5 to 15 min; don't try to "talk them down".
- Vaporize, don't burn it with a direct flame (it destroys the DMT and produces toxic gases).
- Measure the dose if possible; potency varies a lot from batch to batch.
- Don't combine with stimulants, cannabis or lithium. Screen for a history of psychosis.
Doses (smoked/vaporized)
| Level | Dose |
|---|---|
| Threshold | ~2 mg |
| Light | 10 - 20 mg |
| Common | 20 - 40 mg |
| Strong | 40 - 60 mg |
| Heavy | 60+ mg |
Duration (smoked)
| Onset | 20 - 40 sec | Peak | 2 - 8 min |
| Total | 5 - 20 min | After-effects | 10 - 60 min |
Long-term effects / risks
- Not physically addictive; low physiological toxicity.
- Risk of HPPD with very frequent use.
- Anxiety, delirium or transient psychosis are possible even for experienced users; higher risk with a history of psychosis/bipolar disorder.
Combinations
Dangerous / avoid
- Lithium - seizures and psychosis.
- Tramadol - seizures.
- Stimulants - cardiovascular strain, mania, paranoia.
Caution / useful
- Cannabis - amplifies anxiety and paranoia.
- Benzodiazepine - only for a real adverse reaction (to abort the trip).
Protection and supplements
- No established pre-treatment. Magnesium can reduce physical tension.
- Anxiety beforehand: L-theanine recommended (200 mg) or passionflower/lemon balm help you calm down. No CBD on the same day. Set & setting first.
- Integration (rest, journaling, therapy) in the days after intense sessions.
Ayahuasca
Oral DMT + MAOIOral
Antidepressant washout is non-negotiable
Ayahuasca
Antidepressant washout is non-negotiable
- Medication washout is non-negotiable: SSRIs ~4 weeks before, fluoxetine (Prozac) 6 weeks, tricyclic antidepressants and St. John's Wort 2 weeks. Never stop psychiatric medication without your doctor.
- Tyramine-free diet 24 to 48 h before.
- Screening: rule out schizophrenia, bipolar disorder, serious heart disease and pregnancy.
- Only with an experienced, trusted facilitator (there have been deaths from contaminated preparations).
- Know the signs of serotonin syndrome and be ready to purge (hydrate).
Doses
Varies a lot by preparation. Reference (pharmahuasca): ~50 mg DMT + ~100 mg harmaline. Without lab analysis it's practically impossible to standardize a traditional brew.
Duration
| Onset | 20 - 60 min | Peak | 1 - 2 h |
| Total | 5 - 10 h | After-effects | 1 - 8 h |
Long-term effects / risks
- Low abuse potential.
- Purging (vomiting/diarrhea) is common and expected; risk of dehydration in the heat.
- Deaths are rare, almost always from contaminated preparations (brugmansia/scopolamine) or fraudulent facilitators, not from pure ayahuasca.
- Destabilization in undiagnosed bipolar disorder or psychosis.
Dangerous combinations (because of the MAOI)
- SSRIs / SNRIs / tricyclics / St. John's Wort - serotonin syndrome (fever, rigidity, seizures), potentially fatal. They require a washout.
- MDMA and stimulants (cocaine, amphetamines) - serotonin syndrome and hypertensive crisis.
- Tramadol, DXM, 5-HTP, tryptophan - dangerous serotonergic load.
- Other MAOIs and decongestants (pseudoephedrine) - hypertensive crisis.
- Tyramine-rich foods - aged cheeses, cured/smoked meats, fermented soy, sauerkraut, yeast extracts, wine and craft beer. (Harmalas are reversible MAOIs, lower risk than the irreversible ones, but avoid high loads.)
Protection
- Avoid all serotonergic supplements (5-HTP, tryptophan, SAMe) before and after.
- Electrolytes after purging; light food; integration over the following 2 to 4 weeks.
Changa
Smokable DMT + MAOISmoked
Contains an MAOI: warn whoever smokes it
Changa
Contains an MAOI: warn whoever smokes it
- Warn whoever is smoking: it contains an MAOI. MDMA, stimulants and SSRIs taken hours earlier are not neutralized.
- Sit or lie down before the first hit. A bong is more reliable than a joint.
- Vaporize, avoid direct flame. Don't redose right away (the MAOI prolongs and deepens the effect).
- A safe, enclosed physical space. Know the signs of serotonin syndrome.
Doses and duration
No reliable mg scale (it varies by batch). Reference: about one bong bowl, holding it in for ~20 sec. It takes less DMT than pure freebase because of the MAOI.
| Onset | seconds | Peak | 5 - 15 min |
| Total | 15 - 40 min | After-effects | up to 60 min |
Dangerous combinations (active MAOI)
- MDMA and serotonin releasers - serotonin syndrome.
- SSRIs / SNRIs, 5-HTP - serotonin syndrome.
- Stimulants (cocaine, amphetamines) - hypertensive crisis.
- DXM, tramadol, MXE, NBOMe, lithium - dangerous with the MAOI.
- Cannabis - amplifies anxiety. Tyramine: avoid high loads in the hours before.
Protection
- Same as DMT (integration, rest, no serotonergics).
- Avoid the typical festival pattern: MDMA/stimulants earlier and changa later.

Cetamina
Nunca com álcool, opióides, GHB ou benzos
- Nunca sozinho.
Doses (insuflada)
| Nível | Dose | Nota |
|---|---|---|
| Limiar | 5 mg | |
| Leve | 10 - 30 mg | |
| Comum | 30 - 75 mg | |
| Forte | 75 - 150 mg | limiar do K-hole |
| Pesado | 150+ mg | K-hole |
Duração (insuflada)
| Início | 1 - 3 min | Pico | 15 - 45 min |
| Total | 1 - 2 h | Efeitos residuais | 2 - 12 h |
Efeitos a longo prazo do abuso
A ciência ainda é fraca.
Combinações
Perigosas / evitar

Comum: 1 a 2. Forte: 2 a 5 numa sessão.
Efeitos a longo prazo do abuso
- Álcool, GHB/GBL, opióides, tramadol - depressão do SNC, inconsciência e aspiração.
Efeitos a longo prazo do abuso

Long-term effects of abuse
- Ketamine-induced ulcerative cystitis (ketamine bladder): urgency, pain and blood in the urine; affects ~20 to 25% of frequent users. Stopping early is the only thing that prevents irreversible damage.
- Memory and cognitive deficits.
- Nasal damage with regular use: chronic rhinitis, crusting and nosebleeds, loss of smell, septum perforation with heavy use. Ketamine numbs the mucosa itself: snorting it barely hurts, so the pain warning is missing, and with the high, repeated volume of a typical session the damage builds up without you feeling it.
- Psychological dependence and compulsive redosing.
- "Permatolerance": reports (mostly anecdotal) of tolerance that never fully recovers, linked to heavy, frequent use. The science is still weak.
Combinations
Dangerous / avoid
- Alcohol, opioids, GHB/GBL - respiratory depression, vomit aspiration.
- Benzodiazepines - sedation and unexpected unconsciousness.
- Grapefruit juice (oral) - can double the concentration.
With caution
- Stimulants - raised blood pressure, risk of falls.
- Cannabis / psychedelics - they intensify it; low doses, go slow.
Protection
- Hydration recommended before and after (dilutes the metabolites in your urine). No cost and no risk.
- The only proven protection is low doses, low frequency and stopping at the first urinary symptoms. There is no antidote.
- Some people take NAC or green tea (EGCG) for the antioxidant effect. There is a rationale and there are animal studies, but no proof in humans, and they are no substitute for using less. Low risk if you want to take them, but do not count on protection.
Nitrous oxide (N2O)
Dissociative / inhalantBalloon
Sit or lie down: fainting is sudden
Nitrous oxide (N2O)
Sit or lie down: fainting is sudden
- Always use a balloon, never straight from the charger (cold burns).
- Sit or lie down - fainting is sudden and comes without warning (falls kill).
- Never a bag or mask over your face. Never in an enclosed space (car, tent).
- Breathe fresh air between balloons. Never use alone.
- Tingling or numbness in your hands/feet = stop and see a doctor (B12 neuropathy).
- Vegans, vegetarians and anyone with low B12 are at serious risk even with light use.
Doses and duration
Light: 1 charger/balloon. Common: 1 to 2. Strong: 2 to 5 in a session. Large tanks ("galaxy gas") = the equivalent of many chargers and the highest risk of B12 depletion and asphyxiation. There is no safe dose in a space without oxygen (N2O displaces oxygen).
| Onset | 5 - 10 sec | Peak | 15 - 30 sec |
| Total | 1 - 5 min per inhalation |
Long-term effects of abuse
- Vitamin B12 depletion: neuropathy (numbness), spinal cord degeneration, anemia, cognitive decline.
- Hypoxia and brain damage (especially with a bag or in an enclosed space).
- Psychological dependence (the ultra-short duration leads to compulsive use).
- Cold burns on the lips, mouth and throat.
Combinations
- Alcohol, GHB/GBL, opioids, tramadol - CNS depression, unconsciousness and aspiration.
- Any use with a bag or mask over your face - asphyxiation (the main cause of direct deaths).
- Cannabis / psychedelics - they intensify it (briefly); sit down and have someone nearby.
Protection and supplements
Lóló / cheirinho da loló
Solvent inhalantInhaled
Never alone, never in a closed space
Lóló / cheirinho da loló
Never alone, never in a closed space
- Never alone (loss of consciousness is sudden and you can choke or fall).
- Never in an enclosed space (car, room, tent): lack of oxygen.
- Do not run, dance or exert yourself during or right after - adrenaline on a sensitized heart causes fatal arrhythmia.
- Avoid frights or emotional shocks right after (same mechanism).
- Avoid alcohol (it greatly increases liver toxicity).
- If someone loses consciousness: recovery position and 112 right away.
Doses and duration
There is no safe dose and no dose-response data. The effect comes on in seconds and lasts a few minutes per inhalation, which leads to repeated use. Sudden death can occur at any dose or frequency.
Long-term effects of abuse
- Sensitization of the heart to adrenaline (fatal arrhythmia from exertion or a fright).
- Liver toxicity (chloroform generates phosgene) and kidney toxicity; much worse with alcohol.
- CNS depression, loss of consciousness and falls.
- Chronic solvent neurotoxicity (cognitive deficits, cerebellar damage).
- Toxic load of unknown adulterants.
Combinations
- Alcohol - greatly increased liver toxicity and CNS depression.
- Other depressants (opioids, benzos, GHB) - respiratory depression.
- Physical exertion or a fright right after - fatal arrhythmia (the sudden death mechanism).
No combination with lóló can be described as "relatively safe".
Protection
No supplement reliably protects against the acute cardiac or liver event. There is no antidote and no safe dose. The message is: be aware of the risk, never alone, never in an enclosed space, no exertion afterwards.

Duração
| Início | 2 - 5 min | Pico | 30 - 90 min |
| Total | 1.5 - 5 h | Ressaca | 6 - 48 h |
Efeitos a longo prazo do abuso
- Doença hepática (fígado gordo, hepatite, cirrose).
- Síndrome de Wernicke-Korsakoff (deficiência de tiamina, dano cerebral irreversível).
- Vários cancros (carcinogéneo do grupo 1) e doença cardiovascular.
- Canábis - "greening out" (náusea, tonturas, desmaio).

- Re-dose: espera no mínimo 2 h e reduz a dose. Nunca aumentes.
Rotula bem o frasco.
Doses

Duração (GHB)
| Início | 5 - 30 min | Pico | 45 - 90 min |
| Total | 1.5 - 2.5 h | Efeitos residuais | 2 - 4 h |
Efeitos a longo prazo do abuso
- Estimulantes - mascaram a sedação e levam a re-dosar.
Proteção
Nenhum suplemento mitiga a toxicidade. A proteção é medir com precisão, nunca com álcool, e nunca sozinho. Se há dependência, o desmame é médico.
Benzodiazepinas
DepressorOral

Benzodiazepinas
Doses (oral)
| Nível | Diazepam | Alprazolam |
|---|---|---|
| Limiar | 2.5 mg | 0.25 mg |
| Leve | 5 mg | 0.25 - 0.5 mg |

Duração (diazepam)
| Início | 15 - 60 min | Pico | 1 - 1.5 h |
| Total | 4 - 8 h | Metabolitos ativos | muito mais |
Efeitos a longo prazo do abuso
- Dependência física grave (a abstinência é tão perigosa como a do álcool).

Alcohol
Never with GHB/GBL, benzos or opioids
- Eat before and during; never drink on an empty stomach.
- Alternate every drink with a glass of water.
- Set a limit before you start (willpower drops as you drink).
- NEVER with GHB/GBL, benzos or opioids.
- Unconscious = recovery position. Signs of alcohol poisoning (won't wake up, slow breathing, blue lips, cold clammy skin) = 112.
- Don't try to "sober up" with coffee or cocaine: it only masks it.
Doses (grams of pure ethanol)
| Level | Dose | Note |

Long-term effects of abuse
- Liver disease (fatty liver, hepatitis, cirrhosis).
- Wernicke-Korsakoff syndrome (thiamine deficiency, irreversible brain damage).
- Multiple cancers (group 1 carcinogen) and cardiovascular disease.
- Severe dependence; withdrawal can be fatal (seizures, delirium tremens).
Combinations
Dangerous / avoid
- GHB/GBL, benzos, opioids - respiratory depression, can be fatal.
- Ketamine / dissociatives - unconsciousness and aspiration.
- Cocaine - cocaethylene, more cardiotoxic.
With caution
- Stimulants / MDMA - they mask the sedation, you end up drinking too much.
- Cannabis - "greening out" (nausea, dizziness, fainting).
Protection and supplements
- Thiamine (vitamin B1) essential if you drink heavily or often: prevents Wernicke-Korsakoff syndrome (brain damage from B1 deficiency). 100 mg/day.
- NAC recommended ~30 min before drinking: replenishes glutathione and helps the liver neutralize acetaldehyde (which causes much of the hangover). Mixed evidence, but solid mechanism and low risk. Not a substitute for B1.
- Electrolytes essential for dehydration. Avoid paracetamol while drinking (hepatotoxic).

GHB / GBL
Never with alcohol. Measure with an oral syringe
- Measure with an oral syringe (0.1 ml markings). Never by eye or by "capfuls".
- Know the concentration of your solution. If you don't, treat it as strong and start with 0.5 ml.
- NEVER with alcohol, not even "just one beer" hours earlier. It's the mix that kills the most people.
- Redosing: wait at least 2 h and lower the dose. Never increase it.
- Signs of a "G-hole" (won't wake up, snoring/gurgling, blue lips) = 112. Never use alone.
- GBL burns: always dilute it in water or juice. Label the bottle clearly.
Doses

Duration (GHB)
| Onset | 5 - 30 min | Peak | 45 - 90 min |
| Total | 1.5 - 2.5 h | After-effects | 2 - 4 h |
Long-term effects of abuse
- Severe, fast-developing physical dependence; potentially fatal withdrawal.
- Memory gaps and accumulated blackouts.
- Cognitive deficits with heavy chronic use.
Combinations
- Alcohol - the most common fatal mix at festivals. Even low doses of both lead to unconsciousness and aspiration.
- Benzos, opioids, dissociatives - respiratory depression.
- Stimulants - they mask the sedation and lead to redosing.
Protection
No supplement mitigates the toxicity. Protection means measuring precisely, never mixing with alcohol, and never using alone. If there's dependence, tapering needs medical supervision.
Benzodiazepines
DepressantOral
Never with other depressants
Benzodiazepines
Never with other depressants
- The blackout trap: they cause "delusions of sobriety" (you feel less affected than you are) and amnesia, leading you to redose without realizing. Write down the dose you took.
- NEVER with other depressants. Alcohol + benzos is the pair that kills the most people.
- Know the half-life: yesterday's diazepam is still active today.
- Don't stop abruptly if you're dependent - withdrawal can cause seizures. Taper under medical supervision.
- Test street pills: fake "Xanax" is often laced with fentanyl or novel benzos.
Doses (oral)
| Level | Diazepam | Alprazolam |
|---|---|---|
| Threshold | 2.5 mg | 0.25 mg |
| Light | 5 mg | 0.25 - 0.5 mg |
| Common | 5 - 15 mg | 0.5 - 1.5 mg |
| Strong | 15 - 30 mg | 1.5 - 2.5 mg |
| Heavy | 30 mg+ | 2.5 mg+ |
Duration (diazepam)
| Onset | 15 - 60 min | Peak | 1 - 1.5 h |
| Total | 4 - 8 h | Active metabolites | much longer |
Long-term effects of abuse
- Severe physical dependence (withdrawal is as dangerous as alcohol withdrawal).
- Memory and attention deficits; depression.
- Rebound anxiety and insomnia (worse than where you started, feeding the cycle).
- Paradoxical disinhibition and aggression (especially alprazolam).
Combinations
Dangerous / avoid
- Alcohol, GHB/GBL, opioids - respiratory depression; among the leading causes of death.
- Other benzos - stacking is not safer.
With caution / useful
- Ketamine - sedation and unconsciousness.
- Psychedelics - used to abort bad trips (they reduce the effect).
Protection
No supplement mitigates the toxicity or the dependence. If there's dependence, tapering needs medical supervision (with a long-acting benzo). Always test street pills.
Opióides e canábis
Opióides
Depressor opióideOral, insuflada, fumada, IV
Naloxona à mão. Nunca sozinho
Opióides
Naloxona à mão. Nunca sozinho
- Inconsciente a respirar = posição lateral. Material limpo, não partilhar (IV).
Doses
Efeitos a longo prazo do abuso
- "Green out" (pânico, tonturas, náusea) passa: deita-te, hidrata, come algo, respira devagar.
Doses (THC)
| Nível | Fumado/vaporizado | Comestível |
|---|---|---|
| Limiar | 0.4 mg | 1 mg |
| Leve | 0.4 - 2 mg | 2.5 - 5 mg |
| Comum | 2 - 4 mg | 5 - 10 mg |
| Forte | 4 - 10 mg | 10 - 25 mg |
| Pesado | 10 mg+ | 25 mg+ |
Duração
| Fumado: início | 0.1 - 10 min | total | 2.5 - 5 h |
| Comestível: início | 20 min - 2 h | total | 4 - 10 h |
Efeitos a longo prazo do abuso
- Abstinência: irritabilidade, insónia, ansiedade (2 a 7 dias).
Combinações
Perigosas / evitar
- Estimulantes / álcool - ansiedade; "greening out".
- CBD - pode reduzir a ansiedade e o risco do THC.
Proteção
- CBD recomendado ou estirpes equilibradas THC:CBD reduzem a ansiedade aguda.
- Hidratação e comida no "green out". Evita canabinoides sintéticos por completo.
Opioids and cannabis
Opioids
Opioid depressantOral, snorted, smoked, IV
Naloxone within reach. Never alone
Opioids
Naloxone within reach. Never alone
- Carry naloxone and teach someone how to use it. It's the antidote.
- Test with fentanyl test strips (all street drugs, not just opioids; test several portions).
- Never use alone. Start low, go slow, test a fraction of any new batch.
- Tolerance drops fast after any break (3-7 days, jail, hospital): going back to your old dose is a leading cause of overdose.
- NEVER with other depressants (alcohol, benzos, GHB).
- Unconscious but breathing = recovery position. Clean equipment, never share (IV).
Doses
Highly individual and tolerance-dependent. Street doses are unpredictable: heroin can be 5 to 20% pure or contain fentanyl, which makes any reference dose potentially lethal. That's why the rule is always to test a fraction first.
Signs of overdose
- Pinpoint pupils (very small).
- Very slow or stopped breathing (less than 1 breath every 5-8 sec).
- Blue lips and fingertips; unresponsive; snoring or gurgling sounds.
Long-term effects of abuse
- Severe dependence (opioid use disorder); ever-escalating tolerance.
- Chronic constipation, low testosterone and libido, apathy, depression.
- Immunosuppression; IV use adds sepsis, endocarditis, HIV and hepatitis C.
- High mortality on relapse (tolerance resets but the craving doesn't).
Combinations
- Benzos, alcohol, GHB/GBL, other opioids - respiratory depression; among the most lethal mixes.
- Cocaine / stimulants ("speedball") - they mask the sedation; when they wear off, respiratory arrest.
- Tramadol and pethidine with MAOIs - severe serotonergic reactions.
- Cannabis - little pharmacological interaction, but sedation adds up; use less opioid.
Protection: naloxone
Cannabis
CannabinoidSmoked, vaporized, edibles
Edibles: wait 2 h before redosing
Cannabis
Edibles: wait 2 h before redosing
- Start low, especially with edibles and concentrates (far more potent than flower).
- Edibles take a while to kick in: wait at least 2 h before redosing (the most common mistake).
- Avoid it if there's a personal or family history of psychosis, schizophrenia or bipolar disorder.
- Avoid mixing with tobacco (nicotine dependence). Vaporizing is less harmful to your lungs.
- "Greening out" (panic, dizziness, nausea) passes: lie down, hydrate, eat something, breathe slowly.
Doses (THC)
| Level | Smoked/vaporized | Edible |
|---|---|---|
| Threshold | 0.4 mg | 1 mg |
| Light | 0.4 - 2 mg | 2.5 - 5 mg |
| Common | 2 - 4 mg | 5 - 10 mg |
| Strong | 4 - 10 mg | 10 - 25 mg |
| Heavy | 10 mg+ | 25 mg+ |
Duration
| Smoked: onset | 0.1 - 10 min | total | 2.5 - 5 h |
| Edible: onset | 20 min - 2 h | total | 4 - 10 h |
Long-term effects of abuse
- Cannabis use disorder; brain fog, reduced memory and motivation (reversible).
- Risk of psychosis (higher with potent THC, in adolescence and in predisposed people).
- Cannabinoid hyperemesis syndrome (cyclic vomiting; only stops when you stop).
- Withdrawal: irritability, insomnia, anxiety (2 to 7 days).
Combinations
Dangerous / avoid
- Lithium - seizures and psychosis.
- Synthetic cannabinoids (Spice/K2) - not a substitute, far more dangerous.
With caution / useful
- Psychedelics - strong, unpredictable synergy, panic.
- Stimulants / alcohol - anxiety; "greening out".
- CBD - can reduce the anxiety and risk from THC.
Protection
- CBD recommended or balanced THC:CBD strains reduce acute anxiety.
- Hydration and food when greening out. Avoid synthetic cannabinoids entirely.

Passiflora e valeriana têm a melhor evidência (atuam no GABA).
Se for opióide, dá naloxona.
- Cardíaco (estimulantes, lóló): dor no peito, batimento muito acelerado.
Combination matrix

Managing bad trips and crises
First, always (free and the most effective)
- Change the setting: somewhere calm, cooler, with less noise and light.
- Reassure them: "you took a substance, this is temporary and it will pass, you are safe".
- Sit or lay the person down. Slow breathing (inhale 4 sec, exhale 6).
- Water, calm music and someone sober nearby. Don't leave them alone. Avoid mirrors and crowds.
Pharmacological rescue: benzodiazepines
Effective at stopping panic and acute anxiety. The most commonly mentioned: diazepam (5-10 mg), alprazolam (0.5-1 mg) or lorazepam. (Low rescue doses, general information and not a prescription: start with the lowest dose.)
Important: they reduce fear and anxiety, but do not stop the hallucinations.
No access to benzos: natural alternative
Passionflower and valerian have the best evidence (they act on GABA). Good additions: lemon balm (melissa), L-theanine, ashwagandha, magnesium, chamomile and CBD if available.
It calms, but it's no match for a benzo in severe panic. Always combine it with the techniques in step 1.
When it's an emergency (call 112)
- Serotonin syndrome / overheating (MDMA, stimulants, mixes with MAOIs): very high temperature, muscle rigidity, tremors or seizures.
- Respiratory depression (alcohol, benzos, GHB, opioids): unconscious, unresponsive, slow or irregular breathing, blue lips. If it's an opioid, give naloxone.
- Cardiac (stimulants, lóló): chest pain, racing heartbeat.

Resources in Portugal
Asking for help is safe
Since 2001, drug use and possession for personal use have been decriminalized in Portugal (decriminalized, not legalized: trafficking is still a crime). Seeking medical help in an emergency does not lead to criminal charges for personal use. Police presence at a medical call does not mean the person in crisis gets arrested.
Emergency

For any medical emergency. You don't have to explain what was taken in order to get help.
